Traveling abroad is much easier when you have your trusty laptop, tablet or phone in tow. Your devices allow you to be your own travel agent: scoping out activities, curating restaurants, making bookings and reservations and navigating unfamiliar areas. Plus, with some companies maintaining remote-work flexibility into 2022, you can work a bit while traveling so you don't have to use up all your allotted time off for one trip.

But packing your tech isn't as simple as stuffing it into your suitcase. You also need to find a way to keep it charged, connected to the internet, safe and easily accessible. On top of that, different regions use different types of electrical plugs and voltages; if you haven't brought along the right adapter or converter, your tech is rendered useless. 

I encountered these challenges in May when I traveled from the US to the UK, tagging along with my partner on a business trip. We incorporated both work and leisure into our travels -- a practice becoming increasingly common in the form of the digital nomad lifestyle. To keep our devices functional and secure, we relied on a number of tech accessories. 

Here are the gadgets that enabled us to stay plugged in during our international trip. For even more tips, check out this list of travel tech recommendations from CNET contributor Geoffrey Morrison, an expert traveler.

Before I purchased this handy little case, I would store my small electronics and wires by simply cramming them into my backpack. I'd have to dig around and pray I'd pluck the one I needed from the heaps of tangled chargers without having to dump everything out.

No more of that. Outfitted with pockets and zip-up compartments, the Thule Subterra PowerShuttle is perfectly suited for organizing small tech-adjacent doodads: AirPods, adapters, a webcam, small charging cables, spare earbuds and dongles. 

One particularly helpful feature is a hidden cutout in the exterior pocket that allows a cord to pass through: You can stick a phone in that pocket for easy access while you charge it up using a powerbank you've placed inside the case.

This portable charger saved my iPhone's arse on numerous occasions, adding hours and hours to its battery life. Though the chunky 5.9-inch unit (which weighs just under a pound) bogged down my backpack a bit, it certainly pulled its weight, refueling my phone's weary battery while eliminating the need to duck into a cafe in search of an outlet. 

While traveling, I needed to recharge my phone much more often than I'd expected to. I put a real strain on my iPhone's battery, mapping out public transit routes, cueing up boarding passes, snapping photos, researching nearby attractions and food, scanning QR codes and religiously refreshing the West End's virtual ticket booth for dirt-cheap last-minute seats. I'd gulp each time the battery icon turned red, only to recall the surplus of juice tucked right in my backpack. Then I'd plug my phone into the power bank, zip it away, and pull it out later to find its battery revived.

The power bank takes about two hours to charge my iPhone from empty to 100%. It has two USB-A ports, allowing two devices to charge simultaneously.

Depending on where you're traveling, you'll likely encounter a few of the various electrical plugs and voltages used in different regions of the world. For that reason, Protégé's budget-friendly set caught my eye. Not only does it contain adapters to fit different-shaped plugs, but it also includes a voltage converter.

Voltage converters are key because small appliances in North America are generally built to handle a 120-volt supply. Most other regions power their outlets with a much more powerful 220V or 240V supply: Plug an American curling iron into a British outlet and it will fry. A converter tames that intense voltage to 110-120V, a manageable current for North American appliances. (Some devices, like chargers for phones and laptops, are typically dual-voltage and support both standards out of the box.)

Offering adapters of four different varieties, Protégé promises compatibility with outlets in many countries throughout Asia, Europe, Africa, Oceania, and North and South America. Just plug your regular charger into your current location's corresponding adapter, and you'll be all set to use it like normal. Though the adapters feel cheap and plasticky, they seem to get the job done: The Type G plug helped power our three laptops over a 10-day trip to the UK without issue.

Here's where I ran into trouble. When I read on the box that the converter could work with travel appliances between 50 to 1,875W, I thrust my beloved 1,400-watt Rowenta clothing steamer into my suitcase with hopes of easy dewrinkling in the hotel room. However, I failed to note a key word on the box's instructions: travel appliances. When I tried to plug the steamer into the converter, the converter became searing hot, developed a strange smell and only powered the steamer for a few minutes at a time. Baffled, I checked the instruction manual, and there I read the converter was only built for steamers under 1,000 watts. Whoops — I'd overloaded it. Thankfully, the steamer survived, but it still felt like a close call.

Despite that, this is a useful, affordable pick, especially if you're headed to multiple countries on one trip. Just make sure you're prepared to use the voltage converter only for appliances with a low-enough wattage.

While a multipart adapter set like the Protege is ideal for plugging in one device at a time across multiple regions, this seven-in-one adapter's strength is the opposite: It's excellent for plugging in multiple devices simultaneously within one region. This particular model is designed specifically to use in areas with Type G outlets, like the UK. (There's another version for Type C outlets.)

The seven ports and outlets are efficiently placed, allowing the adapter to accommodate several plugs while remaining relatively compact. Three USB-A ports and one USB-C port are neatly lined up along the front panel, along with three AC outlets (one on each exposed side). While the adapter itself isn't too big, the prongs stick out and don't fold up -- a minor annoyance while packing. Stuffing two of these into our carry-on was difficult. 

I used it for my phone and to recharge a power bank. Other reviewers have said they used it for their headphones, CPAP machines and laptops. Remember, it's not a voltage converter, so if you're going to use it with appliances, make sure they're dual-voltage or otherwise built to handle 220V to 240V. Also, note that the USB-C output (17W) isn't strong enough to charge a MacBook, which needs more wattage. I learned that the hard way.

As one who habitually misplaces important items, I try to keep an eagle eye on my personal tech. So, when I bought a Tile tracker to test, I affixed it to the device I'd shed the most tears over losing: My 2020 M1 MacBook Pro. But I discovered I was duplicating an existing Mac feature. Apple's robust Find My app would have let me track down the Mac from my iPhone, doing effectively the same thing as the Tile, but for free. This also would have been the case for a PC or an Android phone, thanks to similar tools from Microsoft and Google.

I'd still recommend getting some variety of Tile gadget for your tech valuables, though: Maybe you have a pair of headphones you treasure, or maybe you want to keep tabs on pricey accessories like chargers. And having a Tile on your phone or computer isn't altogether useless. Since it's hardware that's separate from your device (unlike the Find My apps), the tracker works even if the device is shut off or out of battery.

I was especially impressed with the ease of Tile's setup process. The app walked me through syncing with Bluetooth and turning on my phone's necessary permissions. I could easily locate the tracker on the app's map and trigger its loud beeping. The sticker stuck firmly onto my laptop case, and although the tracker weathered some scratches, it survived plenty of jostling around in my backpack. 

If I were to go back and redo my initial purchase, I would have bought a variety pack: A Tile Slim for my wallet, a Sticker for my laptop charger, and a Mate for my keys. Another option for Apple devices is the Apple AirTag. It's smaller and more advanced than Tile's lineup, but while Tile lets you both track your phone using the tracker and track the tracker using your phone, the AirTag only allows for the latter. And it doesn't work on Android. Tile, meanwhile, works with both iOS and Android.

There's no clear consensus on whether it's truly a good idea to stick a case on a MacBook, as some speculate it weakens the display hinge. But I've advocated in favor of casing, provided you're using a good-quality case that's durable without adding too much bulk. 

This hard shell from Incase checks all of the boxes. It's sturdy, it's light enough not to put pressure on the display, and, unlike a cheaper case I tested, it fits the laptop snugly with no awkward gaps. 

Even as I shuttled my MacBook around airports and streets, I felt confident knowing it had a little extra buffering. The case brought me peace of mind when a thunderstorm caught us by surprise: The backpack was soaked through, but the computer itself was dry as can be.

I'm not saying the case would have protected the laptop if I'd, say, hurled it into the English Channel. But any protection from the elements will certainly do a computer some good.

Get a webcam, but not this one

So far, I've been able to recommend each of the devices I bought for this trip. I can't say the same for this last one. In fact, I specifically un-recommend it. 

Here's the situation. For all their technological prowess, many high-end computers these days are burdened with shockingly low-quality built-in webcams. (Looking at you, MacBooks.) An external webcam can polish your onscreen appearance -- helpful if you're on a trip where you'll be appearing on a lot of Zoom calls or recording yourself with your laptop.

But if you're going to invest in a webcam, pick a different model than the one I tried out. This supposedly 1080p webcam degraded my Zoom image and audio quality from bad to worse. 

The webcam makes the colors a little more flattering (or perhaps just more saturated), but it seems to replace the former image's graininess with... blur. Not exactly an improvement. The autofocus was the worst offender here, despite being this model's selling point: Even my slightest movements would trigger a dizzying refocusing process that often left me blurry in the end. The mic brought a tinny, crackly quality to my voice. I'll probably throw this out -- I'd prefer my Mac's built-in hardware any day, subpar camera and all.

Wordle: Our Simple 2-Step Strategy

Wordle  has been keeping me up at night. Literally. I almost always stay awake past midnight to solve a new puzzle.

The game has gotten all the more satisfying for me since I developed a new, two-step strategy that really seems to work every time. My win streak is at 32 now, and maybe that's nothing to you, but it's a personal best for me. I thought I'd talk you through my strategy so you can start a winning streak of your own (or keep yours going for days to come).

If you're reading this, Wordle needs no explanation to you. The puzzle was invented by Josh Wardle (Wardle -- Wordle, get it?) and bought for big bucks by The New York Times back in January. The online game gives you six chances to guess a randomly selected five-letter word, and if you get correct letters, they show up in different colors, depending on if they're in the right spot in the word or not. It's addictive and fun.

I stuck to the same starter words for a long time, and they'd usually work for me. I was a longtime user of "ADIEU" to get in a ton of vowels, and I would often move on to "STORY," to get the two other vowels (sometimes Y!) and some other major consonants going. And that strategy is still a solid one. I may go back to it if I feel I'm in a rut.

Hop on the 'TRAIN'

But these days, I'm starting every Wordle round with "TRAIN," and then moving on to "CLOSE."

TRAIN is pretty self-explanatory if you've studied which letters are most commonly used in English words. All five of the letters in TRAIN are among the top seven letters used.

All I know is that nearly every time I use TRAIN as a starter word, I get a couple correct letters, often in the right spot. And it's an easy to remember starter word for sure.

CLOSE as a second choice is interesting. It gets two more vowels out of the way, and C, L and S also turn up in the top 10 most-used letter list. Sometimes, though, I get zero correct letters out of CLOSE. But believe it or not, that helps me, too. Then I'm able to look at the keyboard showing me which letters I haven't yet guessed, and choose a word that uses any correct letters plus any major consonants I haven't yet guessed.

Correct letter, wrong spot

If you've got a correct letter, but don't know which spot it should go in, you can have some fun with your third guess. If you know that you have an F but not where it goes, guess a word with a lot of Fs, like "FLUFF." That way, you can at least determine if there's an F in three of the five spots. Remember, too, that there may be more than one of the same letter used.

X marks the spot

Here's one more oddball tip. If I have just a couple of letters in the right spot, I personally need to see how the word looks. So I type in the correct letters I have, plus Xs for each letter I have no clue about. If I know the word ends in "ON" from my guesses, I type in XXXON. Then I study the unguessed letters in the keyboard and mentally picture them where the Xs are. (I don't enter these words as guesses, I just look at them for ideas, then backspace over them and delete.)

If I know there's another letter, say an B, somewhere in the mix, I might try typing it in all the empty spots -- BXXON, XBXON, XXBON -- and see if I can picture what words work. Remember, the Xs don't stand for X, they stand for "I don't know this letter."

If the Xs make it too confusing, you can also just write down the options using blanks. That might give you a better view of possibilities. I choose an X because I like to type it right into the Wordle grid, and X seems the most like a blank to me. Since these attempts aren't really words, there's no danger of me accidentally hitting enter and wasting a guess by mistake.

There you go. That's my new Wordle process. It may not be the perfect strategy. But I almost always find myself in a good place after two rounds of guessing.

Though still not a household name outside of Asia, China's Xiaomi is gaining in global market share for smartphones , and it's not stopping there. Like so many of its rivals, it's also breaking into wearables, with its debut device being the Mi Band fitness tracker.

Perfect for anyone who wants to try out a fitness tracker, the Mi Band's minor flaws are easily ignored, especially when you consider its exceptionally low price. On sale in China at 79RMB (which converts to around $13, £7.50, AU$13.50), it undercuts other basic models like the Misfit Flash and the Fitbit Zip that retail at around $50 (roughly £3o or AU$60) while still delivering the same features.

Good luck getting one, however. There are no plans yet to release the bracelet for other markets, so you'll have to either get it through an online reseller (and pay a little premium), or hope you manage to snag one while in China at the Xiaomi online store.

Design

Xiaomi keeps the design simple for the Mi Band. There are no buttons you need to press and the Mi Band comes in two parts -- a rubber strap and the fitness module. Once on the wrist, the 13g Mi Band doesn't stay in your mind.

As it's water-resistant -- the Mi Band has a IP67 rating -- you can bring the Mi Band with you in the shower, though the rubber strap is uncomfortable when it's wet. The strap is adjustable, so you can either wear it loose or tight depending on your preference.

The Mi Band lacks a screen. Instead, it has three indicator lights that flash when you do a "look at your watch" gesture. However, the Mi Band is really picky about when it lights up; you'll have to actually have your arm fully extended at your side, then raise it up like you're looking at a watch before it works.

The three lights show your walking progress for the day, so if you set the target to, say, 8,000 steps, when you hit around 5,000 the second light will be fully lit. When you hit your target, the Mi Band will vibrate to alert you while the lights will blink for a few seconds.

You'll need to take out the dongle for charging, but you'll really only need to do this once every 30 days or so. The Mi Band comes with a custom USB cable that attaches to the dongle, and you can plug that into a PC or a USB charger.

Unlike the more fully featured app that comes with the higher-end Jawbone Up24 , the Mi Band app is clean and simple and supports both English and Chinese. There's no option to measure your caloric intake -- instead, the app just shows the steps you've taken and your sleep patterns, such as when you fell into deep sleep.

You can also set an alarm to wake up to, and the Mi Band will buzz at the appropriate time. An early bird alarm is also available and will wake you 30 minutes before your preset time if the band detects that it's the best time to wake you up.

Apart from the two functions mentioned above, the Mi Band app also adds a sit-up and jump rope tracker. You'll need to manually activate this, though, as the Mi Band itself only works for your steps and sleep. There's also an option in the settings menu to let you vote for other sports such as cycling in future updates.

One feature that's limited to only Xiaomi's phones is a proximity unlocker. If you're holding the phone in the same hand that's wearing the band, you won't need to enter a passcode or perform a pattern unlock.

Currently, the Mi Band only works with Android 4.4 devices, and there aren't any third-party apps available that support this. It's also unknown if Xiaomi will ever add iOS support, though there are pictures on Weibo from Xiaomi developers showing the app running on an iPhone 6 .

Battery life

As mentioned, the Mi Band claims a 30-day battery life, and two weeks with the Mi Band have shown me that this looks to be true. The fitness band still had 75 percent left, meaning it could last as long as 2 months instead of the promised 1. This is pretty impressive, given that the more expensive Jawbone Up24 is only rated for a week. However, the Mi Band's rechargeable battery can't compare to the six-month battery life of the Misfit Flash or the Fitbit Zip.

Conclusion

Given the Mi Band's absurdly low price, it's impressive and gets a free pass for its minor flaws. The rubber material used in the band takes a while to get used to, but it's light and not at all unpleasant to wear most of the time.

It's really hard to see how other screenless fitness bands can now justify their exorbitant prices, especially when the Mi Band does the exact same thing but is a lot more affordable. Of course, these fitness trackers do have options such as keeping track of your caloric intake, but these are software-based, via an app, so it's possible Xiaomi may be able to add these in as well in a future update.

Lastly, getting your hands on the Mi Band can be challenging since it's limited to China for now, and until the company starts making it globally available, people in other countries will likely will have to pay a lot more for the current fitness bands. With that in mind, I still heartily recommend the Mi Band, even if you have to get it at a premium from an online reseller. You'll still save quite a bit compared with other brands, and the Mi Band makes for a great entry-level fitness band to try out.

Imagine the money you could make (and fun you could have!) if you flew for profit.

Sadly, drone businesses aren't legal in the United States unless the government makes an exception for you -- or unless you get your own drone license under the FAA's brand-new rules.

Starting Monday, August 29, you can apply for your very own license by passing a multiple-choice test and paying a small fee. No experience necessary!

What, you mean I can't just sell my drone photos already?

Nope! You could get fined $1,100 (or more) for operating your unmanned aerial system (UAS) for business purposes without FAA approval.

What is a Remote Pilot Certificate?

It's the official name for the drone license I've been talking about. With it, you can legally fly drones with intention of making a profit -- either by selling the footage, delivering goods and everything in between. You still have to follow a lot of restrictions when you fly...but more on those in a bit.

Prosumer camera drones like the popular DJI Phantom start at around $500 -- but the sky's the limit on price.

What if I'm just flying for fun?

You don't need the certificate...but you do still need to register your drone if it weighs over half a pound (0.55lb to be precise).

Oh, but if you're a drone racer -- weaving through obstacles with a nifty set of first-person-view glasses on your head -- you'll still need that Remote Pilot Certificate. At least that's what the FAA tells Forbes.

OK, I want to make money. What do I need to get my license?

  • Be at least 16 years old
  • Have a valid government-issued picture ID with your name, address and signature
  • Make an appointment to take the knowledge test at a test center near you
  • Pay $150 to the testing firm
  • Study
  • Pass the test
  • Wait up to 48 hours for your test score to get uploaded
  • Apply for your Remote Pilot Certificate (using the code from your successful test as proof)
  • Complete a TSA background check
  • Print out a temporary Remote Pilot Certificate (while you wait for the real one to be delivered by mail)
  • Register your drone (maybe do this first?) and start flying!

OK, that does sound like a lot. Let's break it down.

How do I get an appointment for a Remote Pilot Certificate?

You need to call, email or submit an online request form with one of the two companies supervising the test -- not the test center.

CATS (Computer Assisted Testing Service):

  • 1-800-947-4228, then press 3 (Monday to Friday, 5:30 a.m. PT to 5 p.m. PT, Saturday to Sunday 7 a.m. PT to 3:30 p.m. PT)

According to testing centers we called, CATS and PSI handle pretty much everything, including assigning you a test center and accepting your $150 payment.

How long is the waiting list?

In the San Francisco Bay Area, it's not too bad as of August 26. One testing center told us they were booked for two weeks, another said they had openings later in the week.

Is there a testing center near me?

Probably! There are 686 of them across the United States. Check for yourself in the FAA's full, official list of test centers (PDF).

You'll need to call CATS or PSI to get your appointment, though. While the FAA's website says you can just walk in, testing centers in the San Francisco Bay Area told us we needed to arrange everything with CATS or PSI first.

How hard is the test?

You get two hours to answer 60 multiple-choice questions, and you only need to answer 70 percent of them correctly to pass! (You can totally miss 18 questions and still succeed.)

But these aren't exactly easy questions. You'll probably need to study.

Oh yeah? How hard can the questions be?

While monitoring the Cooperstown CTAF you hear an aircraft announce that they are midfield left downwind to RWY 13. Where would the aircraft be relative to the runway? (Refer to FAA-CT-8080-2G, Figure 26, area 2.)

A. The aircraft is East.

B. The aircraft is South.

C. The aircraft is West.

What's the best way to prepare for the test?

The FAA actually offers a free two-hour training course (register here first) to get you up to speed, plus an 87-page study guide (PDF).

But you may need more help than that, because the FAA's course materials don't explain some of the simplest concepts in plain English.

For instance, would you believe you can answer the sample question above without looking at a map? I didn't figure it out until I found this third-party study guide by drone manufacturer 3DR.

I passed the test! Where do I apply for the actual certificate?

  • Register here first (pick Applicant) and fill out your personal information. (You can ignore the Airman Certificate Number and Date of Issuance fields)
  • Log into the site using your new username and password
  • Apply for the certificate here; pick Pilot from the first dropdown menu, then click on the Remote Pilot certification

Now that I've got my certificate, I can fly anywhere, anytime, yes?

Not so much -- as you'll see when you study, there are a lot of rules (PDF) about when and where you can fly your drone.

Unless you get a 107 waiver (and you need to apply for those months in advance), you can't fly:

  • At night
  • Over people
  • Higher than 400 feet (unless you're within 400 feet of a building; skyscraper climbing is OK)
  • Further away than you can see with your naked eye
  • At speeds over 100 miles per hour

Sadly, drone businesses aren't legal in the United States unless the government makes an exception for you -- or unless you get your own drone license under the FAA's brand-new rules.

Starting Monday, August 29, you can apply for your very own license by passing a multiple-choice test and paying a small fee. No experience necessary!

What, you mean I can't just sell my drone photos already?

Nope! You could get fined $1,100 (or more) for operating your unmanned aerial system (UAS) for business purposes without FAA approval.

What is a Remote Pilot Certificate?

It's the official name for the drone license I've been talking about. With it, you can legally fly drones with intention of making a profit -- either by selling the footage, delivering goods and everything in between. You still have to follow a lot of restrictions when you fly...but more on those in a bit.

What if I'm just flying for fun?

You don't need the certificate...but you do still need to register your drone if it weighs over half a pound (0.55lb to be precise).

Oh, but if you're a drone racer -- weaving through obstacles with a nifty set of first-person-view glasses on your head -- you'll still need that Remote Pilot Certificate. At least that's what the FAA tells Forbes.

OK, I want to make money. What do I need to get my license?

  • Be at least 16 years old
  • Have a valid government-issued picture ID with your name, address and signature
  • Make an appointment to take the knowledge test at a test center near you
  • Pay $150 to the testing firm
  • Study
  • Pass the test
  • Wait up to 48 hours for your test score to get uploaded
  • Apply for your Remote Pilot Certificate (using the code from your successful test as proof)
  • Complete a TSA background check
  • Print out a temporary Remote Pilot Certificate (while you wait for the real one to be delivered by mail)
  • Register your drone (maybe do this first?) and start flying!

OK, that does sound like a lot. Let's break it down.

How do I get an appointment for a Remote Pilot Certificate?

You need to call, email or submit an online request form with one of the two companies supervising the test -- not the test center.

CATS (Computer Assisted Testing Service):

According to testing centers we called, CATS and PSI handle pretty much everything, including assigning you a test center and accepting your $150 payment.

How long is the waiting list?

In the San Francisco Bay Area, it's not too bad as of August 26. One testing center told us they were booked for two weeks, another said they had openings later in the week.

Is there a testing center near me?

Probably! There are 686 of them across the United States. Check for yourself in the FAA's full, official list of test centers (PDF).

You'll need to call CATS or PSI to get your appointment, though. While the FAA's website says you can just walk in, testing centers in the San Francisco Bay Area told us we needed to arrange everything with CATS or PSI first.

How hard is the test?

You get two hours to answer 60 multiple-choice questions, and you only need to answer 70 percent of them correctly to pass! (You can totally miss 18 questions and still succeed.)

But these aren't exactly easy questions. You'll probably need to study.

Oh yeah? How hard can the questions be?

While monitoring the Cooperstown CTAF you hear an aircraft announce that they are midfield left downwind to RWY 13. Where would the aircraft be relative to the runway? (Refer to FAA-CT-8080-2G, Figure 26, area 2.)

A. The aircraft is East.

B. The aircraft is South.

C. The aircraft is West.

What's the best way to prepare for the test?

The FAA actually offers a free two-hour training course (register here first) to get you up to speed, plus an 87-page study guide (PDF).

But you may need more help than that, because the FAA's course materials don't explain some of the simplest concepts in plain English.

For instance, would you believe you can answer the sample question above without looking at a map? I didn't figure it out until I found this third-party study guide by drone manufacturer 3DR.

Not so much -- as you'll see when you study, there are a lot of rules (PDF) about when and where you can fly your drone.

If the pandemic has taught us one thing, it's how to take our health into our own hands. 

We've become our own triage nurse, analyzing a sore throat with such urgency that, in another time, would've been considered a little obsessive. We've been asked to monitor our temperatures and even become citizen public health surveyors with the help of at-home COVID-19 tests. But one day (hopefully soon), the consequences of leaving the house with a sore throat won't mean we're risking someone's life. Soon, our physical health will remain a core piece of our well being, but we'll shake the neurosis of a pandemic mindset – hopefully, keeping our newfound sensitivity to public health and a desire to not harm others in the process.

But will our health care system?

"The pandemic accelerated a lot of changes that were kind of percolating in the background," says Matthew Eisenberg, associate professor of health policy and management at Johns Hopkins Bloomberg School of Public Health. Eisenberg studies how neoclassical economics ("supply and demand") applies to health care. While COVID-19 "catalyzed" many of the changes -- and inequities -- already budding in health care, he said, it will be up to policy makers as well as the supply-and-demand cycle of health care to decide what sticks and what doesn't.

Telemedicine: a thing of the past, or the future?

Video-calls-as-doctor's-visit wasn't a tool created because of COVID-19, but the pandemic has transformed it from an obscure practice to the new way to do health care. Importantly, policy changes made during the pandemic helped knock down some barriers for telemedicine access, and helped providers get paid for it.

Private insurance companies as well as public payers (i.e. Medicare) relaxed their policies on telemedicine reimbursement for health care providers because of COVID-19. As more health care providers get paid for telemedicine (which gives them incentive to provide it), the more supply there is for patients, Eisenberg says. 

"Prior to the pandemic, the only way a Medicare provider could be reimbursed for telemedicine would be if a patient was in a rural area where they could not physically travel to a provider," he says. "Even then, they had to go to a specialized facility and do the telemedicine at some out-patient facility's computer." 

Even through a computer screen, there are roadblocks to accessing health care. Before COVID-19, some patients, depending on where they live and what medical condition they have, would need to drive across state lines to access a specialist (which requires an amount of time and money many patients don't have). The loosening of interstate licensure laws during the pandemic has allowed people to connect with a doctor miles away, and even fill a prescription across state lines. 

Dr. Megan Mahoney is a family medicine doctor and the chief of staff at Stanford Health Care. Stanford Health Care, along with many other providers and organizations, have advocated to keep those restrictions loose once COVID-19 is no longer a public emergency, and the emergency rules no longer apply.

"We have noticed that there are states that don't have a single pediatric endocrinologist," says Mahoney. These specialists treat children with diabetes, for example. "We have a whole team of pediatric endocrinologists."

But in order to participate in telemedicine, you need an internet connection. Mahoney called the bipartisan infrastructure bill, which has a $65 billion budget for expanding internet access to rural communities and helping families pay their internet bill, a "tremendous" help in health care access. In the new virtual health landscape, access to broadband is a "social determinant of health," she says. Some policies and benefits put into place during the pandemic to help families access the internet, like the Emergency Broadband Benefit, were temporary. As broadband continues to mold in its form as a public good, its relationship to health care access will only strengthen.

Sliding into your doctor's DMs

In addition to telemedicine, the pandemic also gave us nearly unlimited access to our doctors' inbox through the patient portal. According to a report published in JAMA, which looked at instant messaging data between patients and their providers from March 2020 through June 2021, the number of patient messages increased, despite fewer patients seeking care in some specialties.

"The sheer demand that we're seeing is very much a testament to the patients' desire for this new channel of care," Mahoney says.  

Even older patients, whose relationship with technology sometimes gets a bad rap, are sending their doctors messages and embracing telemedicine, she says. 

"That was what propelled and accelerated the transformation," Mahoney says. When elderly people, who were originally reluctant to use telemedicine, were forced to use it in order to get care during the lockdown, "that helped them get over that hurdle." 

"What I've noticed is the digital divide, while we do need to be aware of it, it can be overcome and sufficiently addressed through additional education," she says. 

Some of that education for patients requires medical assistants to take on tech support roles. In addition to taking blood pressure and temperature when patients come into the room, they also need to make sure patients are comfortable signing into their patient account and feel comfortable with the technology, according to Mahoney. 

That shift in the patient-provider dynamic, and more direct access to care, is necessary to maintain a system Mahoney says can help people get early intervention and, hopefully, prevent visits to the emergency room.

Many of the messages Mahoney receives from patients involve correcting misinformation patients have heard about COVID-19 -- the type of preventative, education-based work that the current health care structure "does not support," she says. For example, sustaining a more thorough patient-doctor messaging system would require providers be paid for their time consulting with patients off-hours. It also requires online communication to be in the patient's language – a barrier for many people in the US who don't speak English or speak it as a second language. 

"I hope that health care can keep up with this cataclysmic shift that's happening," Mahoney said. "It will have to."

There are arguments against telemedicine as the end-all-be-all. Dr. Thomas Nash, an internist in New York City, told The New Yorker in a June 2020 report that though telemedicine is "doable...I worry that it's going to delay a good exam, and get in the way of deeper interactions between people and their doctors." The informal setting of telehealth may also be less likely to pick up on big issues which routine in-person exams would normally detect, such as high blood pressure, California Healthline reported. And it's more difficult to build an open relationship with your doctor through a screen than it is when you're sitting in their office.

But that also assumes people had a relationship to lose in the first place. As of Feb. 2019, one year before the pandemic began, about one fourth of all adults and half of all adults under 30 didn't have an ongoing relationship with a doctor, according to a report from the Kaiser Family Foundation. This is also a group that shows a strong preference for telehealth, and is the target audience for pre-pandemic care-on-demand services, including Nurx, which allows people to get birth control prescriptions and other medications online, sister sites Hims and Hers, Curology and more.

The great vaccine race 

Scientists impressed the world by moving quickly to develop highly effective COVID-19 vaccines in record time – doses of Pfizer and BioNTech's vaccine, with Moderna's authorized right behind it, were available to the first round of eligible adults in the US less than a year after the the country went into lockdown. According to Nature, the fastest anyone developed a vaccine was for mumps in 1960, and that took four years from development to approval (Pfizer's vaccine for people age 16 and up has full approval by the US Food and Drug Administration, while Moderna and Johnson & Johnson have emergency use authorization). While there's much left to be desired about how the vaccines are distributed and accessed by populations in countries outside the US (only 8.9% of people in low-income countries have had a coronavirus shot), an estimate from the Yale School of Public Health reports the vaccines have saved about 279,000 lives and prevented 1.25 million hospitalizations a s of early July 2021. 

Part of the reason the vaccines were developed so quickly was because research on the technology they use was already underway (the mRNA vaccines were developed using information from HIV research). While the global society has shown we can be very efficient at producing effective and safe vaccines, don't get your hopes up too high that it'll happen that fast again, says Michael Urban, an occupational therapist and program director at the University of New Haven.

"The thing people have to remember is that the federal government pumped tons and tons of money into this development," says Urban. "Globally, not just the United States." 

One reason for that is because COVID-19 had such a prominent impact on our economy. "The fact that this [vaccine] came out is because this is disrupting the fabric of life," Urban says. "How we make money, how we engage with people – how we enjoy our lives."

While it's tempting to hope that because scientists banded together to create a vaccine for COVID-19 and the US government helped fund much of that work it will usher in more resources to find preventative measures and treatments for other diseases, it's unlikely. The incentive for the government to subsidize research and development of treatments for other things that are more individualized, such as cancer or HIV, Urban says, might not be as strong, which leaves it up to the drug companies themselves. And without a public health emergency as transmissible and widespread as COVID-19, it's unlikely drug companies will pour quite as much time and effort into finding treatments.

And when addressing a drug company that profits "billions off of cancer treatments," for example, is it really in the best interest of the company to find an effective preventative measure? 

"If they can do one shot and get rid of cancer, is it really in their best interest?" Urban says. "I hate to say that," says Urban.

Two steps forward, two steps back

In addition to propelling us into trends that've been helpful in health care, the pandemic has magnified our shortcomings and has disproportionately affected the same people who have been mistreated by the medical system for years. Black and Hispanic Americans have been hospitalized with COVID-19, and died from the disease, in much greater numbers compared to white Americans.

Dr. Shantanu Nundy, a primary care physician and author of the book Care After Covid: What the Pandemic Revealed Is Broken in Healthcare and How to Reinvent It, told NPR in a May 2021 report that the pandemic scramble to find a testing site, get a vaccine appointment or access preventative care exposed those who might not have ever experienced it to the perils of health care. 

"The pandemic magnified long-standing cracks in the foundation of the US healthcare system and exposed those cracks to populations that had never witnessed them before," said Nundy in the NPR interview.

Another weak spot exposed because of COVID-19 was the US public health response, and its subsequent communication to the public about what to do when you're sick. When the pandemic struck, public health agencies were relying on "old methodologies" in terms of quarantine requirements and testing rules for COVID-19, Urban says. Compared to other countries, we have issues with containment and quarantine restrictions that don't always prevent people from spreading the virus, he says. The CDC's latest isolation guidance for people who test positive for COVID-19, for example, has been criticized by some for being too relaxed and not requiring a negative test.

In the US, there's a one-and-done mentality. "You do a one-time test, you're cleared," Urban says. "Have a nice day." 

When the next pandemic happens, he says the US is likely still not set up with the structure and tools needed to respond appropriately to a public health emergency. "We didn't learn from the Spanish flu," Urban says. 

An early December report from the Global Health Security Index, an assessment of health security across the globe developed by the Johns Hopkins Center for Health Security and the Economist Impact, backs that up. According to the report, 195 countries across the globe are "dangerously underprepared for future epidemic and pandemic threats, including threats potentially more devastating than COVID-19." 

But importantly, the blame isn't solely on public health agencies, Urban says. The CDC, for example, is "under pressure" to get people back to work and everyday life, Urban says. To do so, the agency has to work within US federal law and the vastly different state and local laws which govern what we can and can't expect people to do.

Looking forward

As we move away from the immediate threat of COVID-19, our appreciation for mental health care is likely to stay. Eisenberg says that we may see specialized mental health services, including some practices that are virtual-only, and some that are a hybrid of in-office and virtual visits. There may also be a shift away from medication treatments for mental health conditions and more provider-focused psychotherapy, Eisenberg finds. 

"It's a small shift, but that could have big implications down the road," he says.

While there are structural and policy changes needed to ensure everyone has autonomy over their health, the pandemic has shifted the way care providers approach health care. Now more than ever, there's an emphasis on public health. 

In an interview with the American Medical Association, Nundy explained the framework he believes is necessary to progress health care after the pandemic. Through the course of the pandemic, Nundy said, doctors "built a muscle" for operating with public health in mind. 

"Let's take that muscle and let's start applying it to diabetes, let's start applying it to mental health," Nundy said. "So much more is possible."
